Criss-cross heart with dextrocardia and transposition of the great arteries: a rare pathology.

چکیده
A 16-year-old male patient who underwent bilateral modified Blalock-Taussig shunt operation at the age of 6 was admitted to our clinic with cyanosis. The oxygen saturation was 60% at time of admission. Echocardiography and computed tomography revealed situs solitus dextrocardia with concordant atrioventricular (AV) discordant ventriculoarterial relation and obstructed bilateral Blalock-Taussig shunts (Figures 1A-D, Video 1*). The posterior left AV valve was related to the morphologic left ventricle located rightwards where pulmonary artery arouse. The anterior right AV valve was related to morphologic right ventricle located leftwards where aorta arouse. Additionally severe pulmonary stenosis and a large ventricular septal defect were demonstrated. Criss-cross AV connection is a very rare cardiac pathology, accounting for less than 0.1% of all congenital heart defects. The morphological basis of the criss-cross heart is the rotation of the ventricular mass along its major axis, which may associate any malformation described in cardiac segments, resulting in various connections between the atria, ventricles, and great vessels.Patients with criss-cross heart usually have ventricular septal defects, transposition of the great arteries, double outlet right ventricle, hypoplastic right ventricle, pulmonary stenosis, and tricuspid hypoplasia. Other associated defects, although less frequent, are straddling mitral or tricuspid valve, subaortic stenosis, aortic arch obstruction, mitral stenosis, and inversion of atria. Rarely, patients may be asymptomatic if there are no associated defects. More than 90% of cases with this pathology were levocardic, and to the best of our knowledge, criss-cross AV-related transposition of the great arteries in a dextrocardic heart is rare. Thus, we want to share the demonstrative views of this rare pathology. 91

A 33-day-old Infant with the Transposition of the Great Arteries; A Rare Case Report
Dextro-transposition of the great arteries (d-TGA) is the one most common cyanotic congenital heart disease in neonates. The discordant ventriculoarterial arrangement results in parallel circulation, it so is vital to understand the management. We report a rare interesting but critical case of 33-day-old boy who developed cyanosis and had transposition of great arteries combined with interrupte...
متن کاملPercutaneous tricuspid valve repair using MitraClip® for the treatment of severe tricuspid valve regurgitation in a patient with congenitally corrected transposition of the great arteries
In selected patients, mitral valve repair using MitraClip® (Abbott, USA) is a relatively safe and well-tolerated treatment for significant mitral regurgitation [1–3]. We describe a 56-year-old female with congenitally corrected transposition of the great arteries (ccTGA) and dextrocardia (Fig. 1a) with recurrent episodes of heart failure caused by a combination of systemic (right) ventricular f...

To the Editor: In a recent issue of Circulation, we saw the interesting images concerning magnetic resonance angiography in a patient with criss-cross heart.1 In the description of the case, the authors correctly report the atrial situs (solitus), the position of the ventricle (inverted), and the connection of the great arteries (discordant with L-position of the aorta). This is the most freque...
متن کاملSurgical repair of both atrioventricular valves in congenitally corrected transposition of the great arteries with dextrocardia.
Patients with congenitally corrected transposition of the great arteries (CCTGA) have a morphologically right ventricle (mRV) that supports the systemic circulation. In these patients, regurgitation of the systemic tricuspid valve (TV) is one of the strongest risk factors for congestive heart failure and mRV dysfunction. Herein, we report the case of a patient with CCTGA plus dextrocardia who u...
متن کاملResynchronization therapy transvenous approach in dextrocardia and congenitally corrected transposition of great arteries.
Cardiac resynchronization therapy (CRT) is an acknowledged treatment for advanced heart failure in acquired dilated cardiomyopathy, resistant to pharmacotherapy. Although there are no therapeutic standards regarding heart failure originating from congenital heart defects with systemic right ventricle, a number of CRT implantations by transvenous approach in congenitally corrected transposition ...
